Forms available at RTOs free of cost

SALEM: People can obtain ten types of forms from the Regional Transport Offices (RTO) free of cost. They are: Form 1 A (Medical Certificate), Form 2 (Learner’s Licence), Form 4 (Test Form), Form 9 (Driving Licence Renewal Form), Form 29 (Notice of transfer of ownership of a motor vehicle) Form 30 (Report of transfer of ownership of a motor vehicle), Form 34 (Hypothecation endorsed), Form 35 (Hypothecation cancelled), Form CFA (Fresh Fitness Certificate) and Form LTVA (Authorisation to Drive a Transport Vehicle).

The forms can be obtained from the RTOs from 10 a.m. to 1 p.m., a press release from Regional Transport Officer P. Suresh said.
